PET RELEASE
In case of emergency, the owner designates "Dog Vacations With Mimi" as agent and understands that "Dog Vacations With Mimi" will do whatever is appropriate for the well being of the pet's health while in our care. Further, if in the eventuality services of a veterinarian are required, the owner extends permission for treatment, as well as responsibility of payment for treatment.
PHOTO USAGE AGREEMENT
You hereby grant to Rhonda and Ken Bradley, d/b/a “Dog Vacations With Mimi “, 6271 Ladd Road, Franklin, TN 37067, the right, Privilege and license, in perpetuity, to manufacture and distribute the photograph(s) of your dog(s) for the purposes of any and all kinds of promotion, publicity, merchandising, licensing,and/or books, calendars, etc. that may derive from these photos. You further agree there will be no compensation to you for the use of any and all photos Mimi takes of your dog(s) for her use. You warrant and represent that you have the right and power to enter into and fully perform this agreement, that the use of photos by Mimi or its license for the purposes set forth herein will not violate any law or infringe any rights of others, and that Mimi will not be required to make any payments in connection with the photos or its use. You will indemnify Mimi and any license of Mimi against all claims, damages, liabilities, and expenses (including reasonable council fees and legal expenses) arising out of any breach of warranties. This agreement contains the entire understanding of the parties and will be governed by the laws of Tennessee applicable to the contract entered into in Tennessee and entirely performed there. No change or modification of this agreement will be binding unless it is made by an instrument signed by you and an authorized signatory of MIMI. MIMI may assign its rights under this agreement in whole or in part.
